Post-Acute Services

Forest Hill's Post-Acute Care Unit is a dedicated recovery and rehabilitative setting for patients recuperating from an injury, surgical procedure or serious illness.

Post-acute residents receive regular nursing services and daily rehabilitative therapy programs. Forest Hill provides physical, occupational, speech, and respiratory therapies as needed. Post-acute care is personalized with both safety and length of stay in mind to allow residents to recuperate as much as possible while minimizing time away from home.

It is our expectation that our patients will return to their homes following discharge from Forest Hill. You can expect to receive the most professional and compassionate care possible, as well as any training you may need, so you can return home and avoid re-hospitalization. Forest Hill is ranked in the “Centers of Medicare & Medicaid Services (CMS) Top National Performers for Patients returning to the Community.”

In 2018, Forest Hill’s discharge to home rate was 73% - a 25% improvement over the national average.

Respite Care

Forest Hill understands the demands of being a primary caregiver. Respite care at Forest Hill eases that burden by placing your loved one in the care of our trained professional staff for a short period of time. Caregivers in need of a break should contact our Admissions department to learn more about respite care services at Forest Hill. Our qualified staff will provide treatment and supervision while you enjoy some well deserved time off.


Forest Hill residents in need of dialysis receive treatment at several nearby facilities. Our staff will help you arrange transportation to and from dialysis, ensure you are up and ready when transportation arrives, and send you on your way with a bagged lunch.
